The Workforce Incentive Program (WIP) – Doctor Stream is an Australian Government incentive designed to support doctors working in regional, rural and remote primary care locations classified as MM3 to MM7.
For GPs, WIP can provide a valuable additional income stream on top of regular billings or salary, particularly for doctors building a long-term career in rural general practice.
This may include:
- VR GPs
- Non-VR GPs
- General Registration doctors on approved GP training pathways
- GPs working across mixed clinic, hospital, urgent care or community settings
If you are considering a permanent GP role in a rural or regional community, WIP can significantly improve the overall value of the opportunity.
For eligible doctors, annual payments may increase the longer you remain active in an eligible location, with higher payments available in more remote MM6 and MM7 locations. VR GPs may be eligible for the full incentive amount, while non-VR doctors not generally receive 80% of the WIP payment value.

How Do You Qualify?
To make a quarter “active”, doctors need to meet one of the following activity thresholds:
Option 1: Central Payment System
Bill at least $6,000 in eligible MBS services during the quarter.
Option 2: Flexible Payment System
Complete at least 21 eligible sessions during the quarter.
One session is three hours, meaning 21 sessions equals 63 hours per quarter.
When Do Payments Start?
For MM3–MM5 locations, doctors need eight active quarters before receiving their first payment.
For MM6–MM7 locations, doctors need four active quarters before payments begin.
This makes WIP especially attractive for permanent GPs who are planning to stay in a rural or remote community for at least 1-2 years.
Do You Need to Be a VR GP?
No. WIP may apply to different doctor groups, including:
Doctors undertaking eligible GP or rural generalist training in MM3–MM7 locations may be eligible for the maximum WIP Doctor Stream payment, provided they meet the program requirements.
Example: Permanent VR GP in MM5
Dr Smith accepts a permanent GP role in an MM5 town.
They work four days per week, bill consistently above the minimum quarterly threshold and remain in the community long term.
By Year 5+, Dr Smith may be eligible for up to $23,000 per year in WIP payments, in addition to their regular GP income.
Example: General Registration Doctor on a Training Pathway
Dr Patel is a General Registration doctor working in an MM6 practice while completing an approved GP training pathway.
Because the location is MM6, payments may begin earlier than MM3–MM5 locations, provided Dr Patel meets the active quarter requirements.
This can make rural training roles financially attractive while also supporting long-term progression toward Fellowship.
Important Things to Remember
Doctors should ensure they are using the correct site-specific provider number for each location, as this is how eligible activity is tracked.
WIP payments are assessed by Services Australia, and mixed work across different MMM locations can be nuanced.
Doctors should keep records of provider numbers, rosters, sessions, billings and any salaried or hospital-based work that may need to be submitted through the Flexible Payment System.
What is the Flexible Payment System (FPS)?
FPS is designed for doctors doing:
- salaried work
- hospital work
- RFDS work
- AMS work
- training/supervision
- mixed billing + salaried work
This allows you to “top up” your payment when Medicare billings alone do not reflect your actual workload.
Example: FPS Top-Up
Dr Patel:
- bills $15,000 via Medicare
- also works hospital shifts
Their MBS billings alone only reach 50% of the threshold.
However, they also complete: 52 eligible sessions
This can effectively top them up toward 52 sessions x 3 hours = 156 hours
Result: Higher WIP payment eligibility.
FPS Application Tips
To use FPS:
You must:
- Register via HPOS
- Log your sessions
- Submit within 6 months of the quarter ending
- Keep rosters/diaries for audit purposes
