This aged care service provider delivers high-quality primary care to aged care residents across Australia, improving the health of aged care residents and supporting RACFs and their staff.
Under their proven model of care, general practitioners access RACFs through existing relationships. The model is based on a year-round clinical health service plan and processes, aiming to provide comprehensive healthcare for your aged care residents.
You will work with nurse practitioners and AHP's to assist with your patients' clinical needs, as desired, allowing you to see more residents.
You will utilise telehealth consults as part of the model of care.
